About Social Work Services

Social work services are available to both clients and their families. The hospice social worker can assist a family in a number of ways, including:
  • Grief support and counseling, including assisting family members in coping with loss issues and care giving demands
  • Assistance in finding community support resources to allow for them to care for their loved one
  • Education regarding the dying process

Bereavement Outreach & Support

Some of the bereavement outreach and support services provided by Jefferson County include:
  • Providing up to 18 months of follow-up services for hospice families, as well as ongoing community bereavement services
  • Making available individual and family counseling
  • Sharing bereavement education and information
  • Scheduling monthly support group meetings for the bereaved
  • Connecting client and family with community resources
